Please call our office and we will be happy to assist in verifying your benefits and coverage.  Please remember, it is also your responsibility to confirm and be familiar with your insurance coverage.

Our office is committed to helping our patients maximize their benefits, and will work with you to achieve the maximum benefit available to you. Payment Options
 
When you arrange your appointment, our friendly staff will advise you of the amount due at time of treatment. If you provide us with the appropriate insurance information we will contact your insurance company to verify eligibility, benefits, and an approximate co-pay amount.  Payment in full is expected at the completion of treatment.  For your convenience, we accept American Express, MasterCard, VISA, and Discover.
